Thank you for inquiring about a puppy from
SilverHawk Australian Shepherds. We are dedicated Aussie lovers who participate in various
dog related events and we breed occasionally to produce what we believe to be a quality
Aussie that conform to the breed standard,
are intelligent and able to do what the Aussie was meant to do - work! We focus on doing
all we can to produce physically and mentally sound dogs.
Through our membership in both local and international Aussie
clubs we are exposed to other points of view and educational programs, and we keep up on
the rules and regulations governing these clubs. We firmly believe in the ASCA Code of Ethics and abide by the ASCA Rules and Regulations. Through our constant
contact with other Aussie breeders all over the world we have the opportunity to share
information and ideas concerning Aussies.
When we have puppies available we ensure that our puppies are only
sold to loving committed homes and we do our best, though this questionnaire and other
means, to ensure a good match between the personality of prospective owners and our
puppies. Our puppies are all home raised and are well cared for and socialized during
their time with us. Puppies going to a new home are not a trained, well-mannered finished
product
they are puppies! They will have accidents and they will get in to mischief,
all part of being a puppy. We expect new owners to finish the job we have started and help
their puppy become a mature, well-mannered Aussie by being responsible for the further
development of the puppy's personality through structured discipline, training, guidance
and most of all, love.
This questionnaire serves the purpose of helping us become better
acquainted with the people inquiring about our Aussies. It serves to present us with a
more complete picture of what you are looking for in a dog and to help us match you as a
potential buyer with a dog that will, ultimately, turn out to be what you are looking for.
This questionnaire in no way constitutes a contract on our part that there will be an
Australian Shepherd available for you, nor do your responses serve as the sole determinant
of whether or not a Australian Shepherd will be offered.
All of our pet puppies are sold on a Limited Registration-without
exception, which means that if the dog is bred, the resulting offspring will not be
recognized or registerable with the ASCA or AKC unless we have agreed, in writing, to lift
the Limited Registration. Puppies sold as pet quality puppies are required to be
spayed/neutered with no exceptions.
Prior to taking the puppy home you will be given an copy of our
"Puppy Info Pak", a wide variety of information on training and care of your
puppy, a list of proper equipment and supplies to obtain prior to bringing your puppy
home, a copy of the pedigree and a sample copy of our sales agreement. At the time you
purchase your puppy you will be asked to sign a completed sales agreement. You will be
given a copy of all applicable registration papers, our health records and any other
information we feel you may need. AKC and ASCA registration application papers will be
given to you to complete and send in.
As much as possible we evaluate each puppy's temperament and
personality by observing them during their development. We attempt to match the puppy to
the buyer to produce the best fit of puppy and family. The order in which you will be
allowed to pick your puppy is determined by the order in which your name appears on our
waiting list along with a deposit being received by SilverHawk. Therefore, if having a
number of puppies to choose from is of importance to you, it is in your best interest to
complete and return this questionnaire in a timely fashion.
We have been notified that people were having trouble with
our online questionnaire so we have chosen to remove it. Please choose below a printable version of
this questionnaire (either pdf or Microsoft Word format) that you can download
to your computer, then fill in and attach to an email, or print off,
complete and
fax or mail to us!
If, after completing the questionnaire and sending it, you
don't hear from me within 3 or 4 days please contact me via email or
phone...I will ALWAYS acknowledge receipt of a questionnaire no matter what,
I have been through the agony of writing to a breeder and never hearing back
and I will not do that to anyone! If I am away, at a dog show or for
any other reason, for more than 2 days, I usually remember to put an
automatic reply on my email so you would get a note telling you when I'll be
back. Please email your completed questionnaire to SilverHawk

We would like to thank you for taking the time to fill in this
questionnaire. The idea behind these questions is that, knowing the pups temperament from
day one, we may see different things in them than you would as a stranger attempting to
pick one. In this way we may be better able to guide you toward a puppy which
realistically suits your experience, needs and goals.
Please
Contact
Trina if you have an
interest in a future puppy!
Please note: We reserve the right to choose who we deem a suitable
home for our pups. In order to be considered as a potential home,
please fill out the questionnaire below and send to

free from any inherited defects, by screening our breeding stock for those problems common
to our breed. However, due to complex genetic interactions and environmental factors
beyond our control, we cannot guarantee that a puppy purchased from us will be completely
free of genetic disorders. If, at any time, your Aussie is diagnosed with any health
problems, genetic or otherwise, we must be notified immediately of the full details so
that we can make every effort to help with the situation. We do guarantee that our puppies
are sold with a complete guarantee against monorchidism, cryptorchidism, Hip Dysplasia,
Iris Coloboma, Cataracts or CEA (Collie Eye Anomoly).
